From risk to precision: the evolving role of genetics in thyroid cancer management (ATA 2015-2025)

Abstract
Genetic analysis has become central to precision management of thyroid cancer. The 2025 American Thyroid Association (ATA) Guidelines greatly expand the role of genetics in thyroid cancer management compared with 2015 Guidelines. Molecular profiling is now used not only for indeterminate nodules but also to guide systemic therapy, refine surgical planning, and identify hereditary syndromes such as PTEN hamartoma tumor syndrome, DICER1-related disease, and Werner or Carney complex syndromes. High-risk somatic alterations, including BRAFV600E + TERT and related mutation combinations, are emphasized for their strong association with recurrence, aggressive behavior, and poor radioactive iodine (RAI) response. A major advancement is the use of somatic profiling to guide targeted therapy in metastatic or RAI-refractory disease, with strong recommendations for RNA-based next-generation sequencing to optimize detection of actionable RET and NTRK fusions. Overall, the 2025 ATA Guidelines reflect a shift toward highly individualized care, using molecular data to optimize risk stratification, treatment selection, and follow-up intensity.
